Missionary Anxiety Assessment

(GAD-7)

Our anxiety assessment form is a widely used, self-administered questionnaire designed to screen for and assess the severity of generalized anxiety. It consists of seven questions, each rated on a scale from 0 (not at all) to 3 (nearly every day), resulting in a total score ranging from 0 to 21. Missionary Depression Assessment

(PHQ-9)

Our depression assessment form is a widely used, self-administered questionnaire designed to screen for and assess the severity of depression. It consists of nine questions, each rated on a scale from 0 (not at all) to 3 (nearly every day), resulting in a total score ranging from 0 to 27. The PHQ-9 is a quick and effective tool for clinical settings and self-assessment, helping individuals and healthcare providers identify and manage depression symptoms. Missionary Stress Assessment

Our stress assessment, created by Katie Ball, provides a comprehensive evaluation of your stress levels over the past two weeks. By running through a list of common stress symptoms, you’ll gain valuable insight into your current stress levels and how they may be impacting your overall well-being.
